Electro-Medical is pleased to offer an electrical safety and preventive maintenance check for your rehabilitation equipment. If you are interested in being included in our schedule please read the following information on our policies and pricing and fill in the form below. We will contact you to schedule an appointment.
Prices
All Ultrasounds and combination units will be calibrated for a charge of $65.00 each. (Exception: Rich Mar Autosound units will be calibrated for a charge of $130.00 each). All exercise weight machines will be examined for safety for a total charge of $65.00 per clinic.
All other clinic equipment (including electrical stimulators, traction units, hi-lo tables, whirlpools, paraffin baths, heat guns, treadmills, stair steppers, hydrocollators, freezers, fluidotherapy units, and compression units) will be examined for electrical and functional safety for a total charge of $65.00 per clinic.
Repairs
Repairs will be done at the discretion of the clinic. If the repair cannot be done in the field, a quotation will be provided for the cost of the repair.
Fuel Surcharge
Due to the rising costs of fuel, all service work done out in the field will incur a $5.00 fuel surcharge.
It is the responsibility of the facility to have all equipment available and accessible for inspection at the time of inspection. Return visits will incur an additional travel charge at the rate of $25.00 per hour.
A complete list of the equipment must accompany this signed agreement to ensure that all equipment will be checked.
If credit is not established, payment is due at the time of the service. Cash, check, or charge is acceptable.
